This Chronic Special Needs Plan (CSNP)—HealthTeam Advantage Diabetes & Heart Care (HMO CSNP)—is only available to residents of Guilford County who have diabetes or chronic heart failure (CHF).

Because it’s a special needs plan, you can enroll any time—you don’t have to wait for the Medicare Annual Election Period (Oct. 15 – Dec. 7).

Friendly formulary—we cover more Tier 1 and Tier 2 (preferred generic drugs and generic drugs) than any other Medicare Advantage plan in the area
