Our Provider Network

HealthTeam Advantage PPO (Preferred Provider Organization) members can visit any provider in our service area. Using in-network providers helps keeps out-of-pocket costs lower and more predictable. If you use an out-of-network provider, make sure they accept Medicare; otherwise, you’ll be responsible for the full cost of services. Except for emergencies or urgent care, it may cost more to visit out-of-network providers.

As you evaluate which plan to select and when you set up appointments for your healthcare needs, you may need to know whether a provider is in the HealthTeam Advantage network.

The formulary, pharmacy network, and/or provider network may change at any time. You will receive notice when necessary. Every year, Medicare evaluates plans based on a 5-star rating system.
