Patient-Reported Health from HOS: A Checklist for Routine Care at Every Visit
As the Health Outcomes Survey (HOS) is mailed to our members starting in July, we would like to remind you what you can do year-round to impact these patient-reported outcome measures. Use the following checklist to ensure you are covering these key topics in every visit.
What the HOS Captures
HOS evaluates how well Medicare Advantage plans help members maintain or improve physical and mental health over a two‑year period. Improving HOS performance requires proactive, documented, and repeated patient engagement focused on:

- physical health and functional status
- mental well‑being
- fall prevention
- physical activity counseling
Why Do These Actions Matter?
Because the survey is repeated two years later, measures change over time, and focuses on patient recall of their care, consistent reinforcement is essential.
The most effective way to improve HOS results is to make these conversations routine at every visit — assess, counsel, document, and follow up.
When patients hear it consistently and remember the guidance, they are more likely to improve their health!
