Medication Management & Specialty Drug Utilization: Partnering for Safe, Effective, and Affordable Care

By Ravinder Chawla, MD

As healthcare continues to evolve, specialty medications and advanced biologic therapies are playing an increasingly critical role in the treatment of complex and chronic conditions. These therapies have transformed care for many patients, offering meaningful improvements in clinical outcomes, quality of life, and disease management.

At the same time, specialty pharmaceuticals represent one of the fastest-growing segments of healthcare spending. As stewards of both clinical quality and healthcare resources, our shared responsibility is to ensure that members receive the most appropriate therapy, at the right time, for the right clinical indication, while preserving affordability, access, and long-term sustainability across the healthcare continuum.

A key component of achieving this balance is the adoption of evidence-based formularies, clinically appropriate use of biosimilars, and step therapy programs. Biosimilars have demonstrated comparable safety, efficacy, and quality to their reference biologic products and represent an important opportunity to expand patient access while reducing unnecessary healthcare expenditures.

Similarly, step therapy protocols are designed to encourage the use of proven, cost-effective therapies before advancing to higher-cost alternatives when clinically appropriate. We encourage providers to remain familiar with formulary requirements, preferred treatment pathways, and available biosimilar options, as doing so can help reduce treatment delays, minimize administrative burden, and improve the overall member experience.

Looking ahead, technological advancements will play an increasingly important role in supporting medication management at the point of care. Enhanced integration between health plans, pharmacy benefit managers, and electronic health records (EHRs) has the potential to provide real-time visibility into formulary status, preferred specialty medications, biosimilar alternatives, prior authorization requirements, and member-specific benefit information directly within the provider workflow.

For example, when ordering a high-cost specialty medication, clinicians may be presented with clinically equivalent biosimilar options, coverage information, and evidence-based treatment recommendations within the EHR. These tools can help:

  • Reduce administrative complexity
  • Support informed clinical decision-making
  • Improve prescribing efficiency
  • Accelerate patient access to appropriate therapies

When exceptions are clinically warranted, thorough documentation of prior treatment history, medication intolerance, adverse reactions, or other medical factors supporting the exception remain essential to facilitate timely review and approval.

Prior authorization requests for specialty medications are most effective when accompanied by complete and accurate clinical documentation. Common causes of delays include:

  • Missing diagnosis information
  • Lack of supporting laboratory or imaging results
  • Incomplete medication histories
  • Insufficient documentation demonstrating inadequate response to first-line therapies

Providing comprehensive clinical records at the time of submission enables our clinical teams to make informed decisions more efficiently and helps ensure that members receive needed therapies without unnecessary delays.

We deeply value our partnership with providers and remain committed to advancing high-quality, evidence-based medication management. By working together to optimize medication utilization, promote appropriate adoption of biosimilars, leverage emerging technologies, and support adherence to clinical guidelines, we can continue to improve patient outcomes while ensuring the responsible stewardship of healthcare resources for the members and communities we serve.
