ER Visit Follow-up for Those with High-Risk Multiple Chronic Conditions

Patients with multiple chronic conditions are among the most vulnerable populations following an Emergency Room (ER) visit. Without timely follow-up care, these individuals face an increased risk of medication errors, worsening symptoms, avoidable hospital readmissions, and gaps in care continuity. Your engagement is critical.

To prioritize follow-up after ER visits, providers should:

    • Identify and address unresolved medical concerns
    • Reconcile medications and reduce adverse drug events
    • Reinforce treatment plans and self-management goals
    • Connect patients to needed specialty and community resources
    • Reduce preventable readmissions and emergency department utilization

Provider Follow-up Checklist

⃞  Schedule post-ER follow-up appointments promptly*

⃞  Review discharge instructions and care plans with patients

⃞  Assess chronic condition management and identify barriers to care

⃞  Coordinate with specialists, care managers, and support services as needed

*If an in-person appointment cannot be scheduled within the recommended 7-day timeframe, consider utilizing RN or CMA outreach through a telehealth or transition-of-care call. This process assesses the patient’s status, reinforces discharge instructions, identifies barriers to care, addresses urgent concerns, and supports timely provider follow-up.

Together, We Can Make a Difference

Every successful follow-up visit is an opportunity to improve health outcomes, strengthen care coordination, and provide patients with the support they need to manage complex health conditions effectively.
