• • Performing Annual Wellness Visits for eligible patients
  • Following up with members who have uncontrolled hemoglobin A1C, and scheduling redraws
  • Documenting member-reported blood pressure readings when appropriate
  • Addressing patient hesitancy around preventive screenings by emphasizing the benefits of early detection and regular testing
