Getting regular dental care is important not only for your teeth and gums, but your overall health.

Hearing
Good hearing is important to your health and safety.
TruHearing providers must be used for routine hearing benefits and hearing aids:
- $25 copay for routine hearing exams. One exam visit per year.
- Up to two (2) TruHearing-branded hearing aids per year.
- Copays for hearing aids range from $299–$799.
- Rechargeable battery options available on select styles ($50 per aid).
- $0 copay for hearing aid fitting/evaluation visits for up to one year after purchase.
Getting Started
- Find a participating TruHearing provider by calling 866-201-9886.
- Schedule your exam. (If you need help, contact your Healthcare Concierge.)
- Go to your exam.
- Order hearing aids if needed. Then go for a fitting and follow-up.
Check Your Hearing Online
TruHearing offers an online tool to check your hearing. This doesn’t replace seeing a provider, of course, but offers a simple screening from the comfort of your home. You will need a pair of headphones for the screening, which takes less than five minutes.

Vision
Healthy eyes and vision are important to your well-being.
- $0 copay for routine eye exams with a VSP provider. One per year.
- $175 annual eyewear allowance for frames or contact lenses.
- $0 copay for lens upgrades (standard progressive lenses and scratch-resistant coating).
Using your vision benefit is easy:
- Create an account at vsp.com. Review your personalized benefit information.
- Find a HealthTeam Advantage provider who’s right for you.
- At your appointment, tell them you have VSP and present your HealthTeam Advantage ID card. There are no claim forms to complete when you use a VSP provider.

Over-the-Counter (OTC) Allowance
Use this quarterly allowance on OTC products and healthy foods.
- You will receive a prepaid debit card in the mail from NationsBenefits.
- Activate your card by visiting HTA.NationsBenefits.com/Activate or calling 877-205-8005 (TTY 711).
- Visit HTA.NationsBenefits.com to download a catalog, see a list of participating retailers, or place an online order.
- Your allowance is $70 per quarter.
- You can roll your funds to the next quarter, but you must use your entire allowance by December 31. Please save your card for future use.

In-Home Support

- $0 copay for up to 60 hours per year of Papa Pal services.

Custodial Care
Personal, professional, non-medical care when you need it most.
Receive non-medical care performed by professional caregivers after a hospital stay or qualifying outpatient procedure.
- Covers up to 20 hours of care after a hospital stay or qualifying outpatient procedure, for a maximum of 60 hours per year, at no cost to you.
- Custodial care must be provided by a professional home health agency or provider. Prior authorization is required.
- Care services must be initiated within 30 days of discharge from inpatient stay or outpatient procedure.
- Qualifying outpatient procedures must be performed at a facility, not a provider’s office, and they do not include outpatient diagnostic tests such as colonoscopies or biopsies.
Transportation
Attending medical appointments and sticking to care plans are key parts of staying healthy.
- Up to 48 one-way rides to or from plan-approved, health-related locations at no cost.
- Up to 100 miles per one-way trip.
- Call SafeRide at (877) 841-1351 to request transportation:
- Rideshare, Ambulatory NEMT, or Wheelchair Van service available
- On-demand service for last-minute needs
In-Home Meal Delivery
Get the support you need with meal assistance.
- Coverage includes up to 42 meals (2 meals per day over a 21-day period) from NationsMarket™.
- Qualifying events include surgery, discharge from an inpatient hospitalization or skilled nursing stay, and some chronic illnesses (chronic heart failure, diabetes, and/or chronic lung diseases).
- Your Care Management team can work with your healthcare provider to coordinate this benefit.
- Please contact your Healthcare Concierge if this benefit is not initiated by your provider after a qualifying event.

Worldwide Travel Coverage
Healthcare coverage while traveling outside the U.S.
Coverage for urgent care or emergency care outside the United States and its territories includes:
- $0 copay for each urgent care visit
- $0 copay for each emergency care visit
- $75,000 maximum coverage amount per year
To use this coverage, follow these steps to be reimbursed:
- Pay for the urgent care or emergency services you receive outside the United States or its territories.
- Get receipts for all services provided.
- When you return to the United States, contact your Healthcare Concierge for instructions on submitting a claim for reimbursement.
- Submit your claim within 60 days of the date you received the service.
Note: Supplemental benefits such as dental, vision, hearing, etc., are not covered outside the United States. Worldwide emergency services and urgent care do not apply to your Out-of-Pocket Maximum.
