Dental Code Quick Reference Guide

Dentist fees can vary, and your plan may cover a procedure at a different amount or not at all. By getting the CDT code from your dentist before the procedure, you can use this guide to confirm what will be covered and what you will need to pay out-of-pocket. This is particularly important for expensive or complex treatments.

Dominion Dental Utilization Review Guidelines

Dominion National’s Utilization Review (UR) guidelines explain the criteria that we use to evaluate pretreatment estimates, pre-authorization requests, and/or claims. The Guidelines were formulated based on generally accepted dental standards and information gathered from practicing dentists, dental schools, insurance companies, and other dental-related organizations.
