Why Code Recapture Matters for Quality Care

Making sure every patient’s chart is accurate helps us give better care and improves Healthcare Effectiveness Data and Information Set (HEDIS) quality scores. “Code recapture” means updating important information at each visit, such as allergies, past surgeries, current and past health problems, and procedures a patient has already completed. 

When this information is missing or outdated, it can look like a patient needs a test or screening that they actually don’t. For example, if a patient had a bilateral mastectomy but it isn’t coded in the chart, the system may show them as “overdue” for a mammogram. This creates extra work for staff and confusion for the patient. The same happens when we don’t record things like a past colonoscopy, a completed diabetic eye exam, or important allergies such as a statin allergy. 

Updating codes helps us see the full picture of the patient’s health. It keeps care plans safe, prevents unnecessary outreach, and ensures quality measures — like cancer screenings or chronic disease followups — are counted correctly.
