For Providers / Required Annual Model of Care Training for C-SNP Providers
The Centers for Medicare & Medicaid Services (CMS) requires that all providers seeing beneficiaries enrolled in a Chronic Special Needs Plan (C-SNP), such as our Diabetes & Heart Care (HMO C-SNP), participate annually in Model of Care (MOC) training, and attest to completion.
