You’re eligible for HealthTeam Advantage PPO plans if:
- You’re entitled to Medicare Part A and are enrolled in Medicare Part B
- You live in one of these North Carolina counties: Alamance, Caswell, Davidson, Davie, Forsyth, Guilford, Montgomery, Orange, Randolph, Rockingham, Stokes, or Yadkin county
- You are a United States Citizen or lawfully present in the United States
You’re eligible for HealthTeam Advantage Diabetes & Heart Care HMO CSNP if:
- You’re entitled to Medicare Part A and are enrolled in Medicare Part B
- You live in one of these North Carolina counties: Alamance, Davidson, Davie, Forsyth, Guilford, Randolph, or Rockingham county
- You are a United States Citizen or lawfully present in the United States
- HMO CSNP only: You have diabetes and/or chronic heart failure (CHF)
